Anti Endomysial Antibodies
Immunology and Allergy Overview / Directory
Purpose of the test
Anti-endomysial antibodies (EMA) of the IgA isotype have, according to some studies, almost absolute sensitivity and specificity in the diagnosis of gluten sensitive enteropathy or coeliac disease (CD) and are considered by many to be the “gold standard” of available CD assays. Other published data suggests EMA is more specific than IgA anti-tissue transglutaminase (tTG), but less sensitive. The discrepancy indicated by such works may reflect the level of expertise of the microscopist. The investigation is compromised in cases of IgA deficiency. In such cases IgG EMA is recommended. As a monitor of dietary compliance EMA may be of use although its disappearance does not signify return to normal villous architecture. We offer EMA by indirect immunofluorescence.
